Developed by the PA Department of Health and the PA Community of Practice on Transition, this valuable tool is available as a free download.
Skill inventory and planning timeline
The document includes a transition skill inventory that emphasizes self determination skills such as understanding personal interests, strengths and needs. Skills related to wellness, safety, self care and advocacy, employment, community participation and postsecondary education are also included.
A general timeline is suggested for key transition steps.
Resources and links
There are resource pages and links on a wide variety of transition topics from health insurance to social security, government agencies, assistive technology and much more. Clear language and simple format make the document user-friendly.
Some links are specific to Pennsylvania. But you’ll find much of the information truly helpful no matter where you live.
