Many transition planning tools address just one area of a young person’s life. Education, for example. Or career planning, or health.
But planning for the future involves all aspects of our child’s life. That’s why it’s important to start with a ‘whole person’ approach to transition planning.
The tools in this section help kids consider what matters most to them in every area of their life. So they can set goals and make plans to build the future they desire.
The format of the tools vary. Some are more general than others. Check them out with your child. Sometimes it’s best to begin with a general tool, and then use a more detailed one. It’s up to you!
