The tools in this section will help you and your child navigate the transition from child to adult health care.

There are resources that will help kids gradually learn to better understand and care for their health needs and communicate with health providers.

You’ll also learn how to plan for changes that will occur when your child reaches the age of majority, transition to adult healthcare providers, and understand potential changes in insurance.

Many of these resources are prepared by, and feature videos of young adults and their parents. You can learn from those who have traveled the path that lies ahead.
